How to Find Your Insurance Information
A Quick Guide for Patients Before Scheduling
To help us verify your benefits and provide the most accurate estimate, please have the following information ready when scheduling your appointment.

Step 1: Find Your Insurance Card

You may have:
✅ A physical insurance card in your wallet
✅ A digital insurance card in your insurance company's mobile app
✅ An insurance card available through your employer's benefits portal

Step 2: Determine if You Have Dental Insurance or Medical Insurance

Dental Insurance
Dental insurance cards usually say:

• Dental

• Dental Plan

• Dental Benefits

• Dental PPO

• Dental HMO

• DMO

Common dental insurance companies include:

• Delta Dental

• Cigna Healthcare

• MetLife

• Guardian Life

• Humana

• UnitedHealthcare

• Aetna
   

Medical Insurance
Medical insurance cards usually say:

• Medical

• Health Plan

• Health Insurance

Medical insurance generally does not cover routine dental treatment, but we may still need it for certain procedures.

Step 3: Locate the Insurance Carrier

The insurance carrier is the company providing your insurance coverage.
Examples:

• Delta Dental

• Cigna

• Aetna

• Humana

• MetLife

• Guardian

• UnitedHealthcare

Example

• Delta Dental PPO

• Insurance Carrier = Delta Dental

Step 4: Find Your Member ID

Your Member ID is your personal identification number with the insurance company.
Look for:

• Member ID

• Subscriber ID

• ID Number

• Member Number
   

Example

• Member ID: ABC1234567

• Member ID = ABC1234567

Step 5: Find Your Group Number

Your employer often provides a Group Number.
Look for:

• Group Number

• Group #

• Group ID
   

Example

• Group Number: 789456

• Group Number = 789456

If you cannot find a Group Number, that's okay—many plans do not have one.

Step 6: Find Your Group Name

Some insurance cards include:

• Employer Name

• Group Name

• Plan Sponsor

• Employer: ABC Manufacturing

• Group Name = ABC Manufacturing

Not every insurance card includes a Group Name.

Step 7: Determine Your Plan Type

Look for the plan name on your card.

PPO (Preferred Provider Organization) - Accepted

• Examples:

  • Delta Dental PPO Cigna PPO

​PPO plans generally allow you to see any dentist, although benefits are usually higher with in-network providers.

HMO / DHMO / DMO - Not Accepted

• Examples:

  • Dental HMO

​These plans often require you to select a specific dental office.

Medicaid
Your card may include:

• Medicaid

• Medicaid Managed Care

• CareSource

• Molina

• Buckeye

• Humana Healthy Horizons

• AmeriHealth

If you are unsure whether your plan is Medicaid, simply tell us the insurance company and we can help identify it.

Step 8: Take a Picture of Your Insurance Card

Please send:

✅ Back of card (if available)

Scheduling Script for Patients
When calling or completing an online form, please provide:

• Insurance Carrier:

  • Example: Delta Dental

• Member ID:

  • Example: ABC1234567

• Group Number (if available):

  • Example: 789456

• Group Name (if available):

  • Example: ABC Manufacturing

• Plan Type:

  • PPO, HMO, DHMO, DMO, or Medicaid

• Primary Subscriber Name:

  • Example: John Smith

• Patient Date of Birth:

  • MM/DD/YYYY

Don't Have Your Insurance Card?
No problem.
Please provide:

• Insurance company name

• Employer name (if applicable)

• Your full name

• Date of birth
